Step 1
Open up photoshop and start a document 100x30 Pixels with a white bg, create a new layer, by going to Layer -> new ->layer.

Step 2
Choose your rectangular marquee tool and use the style "Fixed Size" in the drop down box at the top and make the selection 96x26 pixels, now make a selection in the middle leaving 2 Pixels on each side.

Step 3
Take your gradient tool and choose reflective gradient, it should be your 4th choose at the top selection for gradients.

Step 4
Change your front color to #0e72b2 and your back color to #ffffff, then put your cursor in the middle of your selected box and click down and pull upwards just above your image area. Now go to edit -> stroke, and use the same color as your front color which is #0e72b2.

You should have something like this so far:

Now lets move on.

Step 5
Right click your Layer and go to Blending Options

Step 6
Choose these settings:

Step 7
Take your PEN tool and choose the this selection at the top:

Then take your pen tool and make a selection on the image like so:

Then right click on your peen tool and choose the option that looks like the top of an arrow, should be the last selection, then grab your middle square on your thing like the image above and pull it until it looks like this:

Now if you followed that then right click on the selection like above and choose the option:  Make Selection...

Step 8
Now add a new Layer by going to Layer -> new -> layer

Step 9
while on the new Layer with the area still selected like before go to edit -> fill, and fill the Layer with white, which should be your background color.

Step 10
Look at your layers box and change the Layer style to SOFT LIGHT

Step 11
Add some text, and your done!
