Create a new 200x300px document. Fill layer1 with the color #222222.
Draw a 122x105px rectangular an fill it with #5cb7f0 like this:


Create a new layer and draw a new rectangular with the size 108x16px and the color #4e9dcf. Place it 2pixels from the top of the other rectangular an 2px from the left side of the other rectangular.


Draw a arrow with same color and on same layer as the last rectangular like this:


Take the Eraser Tool and delete equal much as the arrow:


Duplicate the layer and move it down 5 times an you should have something like this:


Add the text you want to have on your navigation with Tahoma 11px #4e4e4e.


Type navigation above the navigationbox.


Choose the Elliptical Marquee Tool and draw a circle like this and press Ctrl+Shift+I to inverse the marking.


Create a new layer. Select the Gradient Tool and fill the marking.


Result:
